This form is for University staff who are having difficulty with Multi-Factor Authentication (MFA) when accessing their University services.

If you still need help, let us know the information below and your query submission will be forwarded to the MFA Support team who will be in touch as soon as possible.

If you are a student, please use the Multi-Factor Authentication (MFA) Query Form for Students.

If you have a lost or broken mobile phone, please try adding a Desktop Authenticator on a computer you have access to.

    

Identity Verification

In order to resolve your MFA issue, we need to verify your identity. You can help us by providing the following pieces of information. This will allow us to help you more quickly. 

If you do not provide this information below we will need to contact you to verify your identity before we proceed.

If we need further verification, we may need to arrange a Microsoft Teams call with you. You shouldn't need to access your own University Teams account for us to arrange this.

  • whether the device is university-managed or personal
  • the name of the computer
  • how long the issue has been occurring
  • what steps you have tried so far
  • any error messages you have received
